Clinical trial design as a living system.

No drug becomes a cure on its own. It becomes one inside a clinical trial: a multi-year, multi-million-dollar process designed to find out whether what was true in a Petri dish is also true in a person. The trial is what turns a molecule into a medicine.

At the center of every trial sits its design; deciding who gets in, what gets measured, and whether the science survives contact with reality. It is the most consequential process in drug development.

For as long as clinical trials have existed, that work has been treated as a craft, not a system. It has carried decades of judgment, expertise, and care; and never the infrastructure this consequential work deserves. The amendments that follow are not the disease. They are the symptom of work that never had a system underneath it.

Which is why we treat trial design as a living system, letting the next trial start where the last one ended.

This is work that doesn't succeed in isolation. We build with the people closer to the problem than we are, across every part of the trial.

Every constraint we encode comes from one of them. Every recommendation we surface cites the trial it came from. Nothing about clinical trial design is solved by a single company, and we are not trying to.

Speed and cost are the smallest of the prizes. The real work is to raise the standard; to recover months of waste, to make trial design worthy of the science it carries, and to recover the medicines whose trials failed not because the molecule did not work, but because the design did not survive the operations around it.

That is what trial design at the center of every cure looks like. That is what we are building, and who we are building it with.
